ELIGIBILITY

• Patients, who are eligible, should contact the practice on 6341 1400 to make a booking to receive vaccination.

 CONSENT FORM

• Patients who are able to, are also asked to come with the patient section of the consent form

On the day of vaccinations patients asked to arrive in clothing that will allow the vaccine to be given in the upper arm, without the need to remove any clothing.

 

Patients who wish to speak to their doctor to make a decision about having the vaccine should make an appointment to discuss this BEFORE booking into the vaccination clinic.

 

The practice will also continue to run the respiratory clinic from the rear of the building to ensure that our town is provided with the best possible care.
