PRACTICE COMMUNICATION POLICY

As a result of the amendments to the Privacy Act, once a patient reaches the age of 16 or is deemed to be able to manage their own medical affairs, we are unable to give any medical information to another person unless previously nominated by you or the patient's legal carer. This can only be accepted if done so by written authority. This practice adheres to a strict privacy and confidentiality policy, without written consent no information will be disclosed to anyone other than the patient themselves. 

 

It is important that if your circumstances change at any time with particular regard to the accessing of your medical information that you please attend the practice and complete another privacy act form. The Practice may at its discretion request patients' to complete an updated patient data sheet and privacy act form. 

Further details regarding information privacy can obtained via the Practice's Privacy policy or via the 1988 Privacy Act.

 

Patients that wish to change any of their details, with the practice, or with regards to the accessing of their medical records may attend the practice and fill out a 'Patient Data Sheet'. Alternatively, patients may access the form via the link below and return the form to the practice.
