Kendal Street Medical Services offer patients a Nurse lead diabetes clinic that is run within the practice.

This clinic aims to: 

• Provide holistic and patient centered care for patients in the the management of diabetes

• Assist patients to develop insights into the lives and lifestyle of people living with diabetes, with the aim of improving their health outcomes

• Provide people living with diabetes information and understanding of the importance of taking an active role in their own healthcare.

• Newly diagnosed diabetes 

• Gestational diabetes education

• Glucometer education 

• Annual cycle or care assessments

• Dietary and physical activity review

• Education & assistance with starting insulin or inject-able medication 

• Carb counting

• Any trouble shooting or questions you may have about your diabetes management. 

• Hypoglycemia (Hypo) Management
